Palliative Care

CareCHOICES Palliative Services knows that a diagnosis of a serious and complex medical condition or illness is a life changing experience that alters your priorities at a moment's notice.

The individual looks ahead at a period of time potentially filled with uncertainty. It may be difficult to know where or who to turn to for expert guidance and assistance.

When undergoing treatment for a complex condition, patients and their families can quickly become overwhelmed with the array of specialists, medical tests, treatments, medications and questions they encounter.

But there is hope and there are answers...

With palliative care through CareCHOICES, patients and their families are surrounded with an extra layer of support by a team of highly specialized palliative medical professionals. The CareCHOICES Palliative Team works together with you to establish and accomplish the goals of maximizing comfort, managing symptoms and maintaining a holistic, personalized plan for care navigation.

Regardless of whether a patient is in his or her own home, an assisted living facility or wherever else the patient may call home, the primary goal of palliative care is to improve the quality of life for the patient and family, to allow time to live every moment to its fullest.

Palliative services vary greatly, depending on the need and current condition of every patient, as well as the benefit coverage offered by each insurance company or medical group.

CareCHOICES offers the opportunity to create care that has the greatest benefit, at just the right time.

Some of the care options available include:

  • Nursing visits to coordinate care.
  • Intensive symptom and comfort management to alleviate pain, fatigue, anxiety, nausea, loss of appetite, sleeplessness and depression, or other complex medical care issues.
  • 24-hour telephone access to palliative care nurses to help coordinate the need for services, consistent communication, and visit support.
  • Trusted care coordination in navigating the health care system.
  • Ongoing education regarding all care options for every stage of wellness or illness.
  • Guidance regarding complex treatment benefit or burden, emergency room admission, or re-hospitalization choices.
  • Physical, emotional, and spiritual support to encourage complete well-being throughout the care continuum.

CareCHOICES has a large, dedicated palliative care specialty team that has been a pioneer on the leading edge of palliative care in the Southern California service area for over five years. We are the trusted palliative expert and resource for post acute care and community based palliative services for many insurance companies and progressive medical groups.

CareCHOICES Palliative Care is appropriate at any age and at any stage of a serious illness, and it is not dependent on a specific diagnosis or prognosis.  Our care may be provided from the time of initial diagnosis or throughout the course of the illness progression, in collaboration with your physician or team of medical specialists.

Your CareCHOICES Palliative Team will work with you while you are undergoing treatments that may cure or reverse the effects of the condition, or the team can guide you if you choose palliative care to assist with symptom management as the condition naturally progresses.

Insurance Coverage

Palliative care is typically considered a specialty service, and insurance coverage for the service varies greatly. However, a shift within the medical community has created a push for increased access to palliative care.

CareCHOICES accepts all forms of reimbursement for services, and our admission specialists are available to explore your coverage options with you and your insurance company. We will make all of the calls so that you don't have to as we determine what supportive care coverage is available for you.

Plan of Care

Working with you and your family, your CareCHOICES Palliative Team helps to ensure you are receiving care that is aligned with your goals, preferences, and values.  We ensure and encourage awareness of your illness progression and promote early discussion about issues surrounding your future health status.

Advanced care planning is a hallmark of palliative care to honor your wishes each step of the way along this journey.

Your CareCHOICES Palliative Team can help you cope with aggressive treatments by providing pain and symptom management to promote your comfort and quality of life.  Your plan of care is personalized based upon your needs, the intricacies of your illness and your desires for care. With advanced care planning, all goals for care will be directed by you and be individualized to match your plans for living life your way, every day.
