With palliative care through CareCHOICES, patients and their families are surrounded with an extra layer of support by a team of highly specialized palliative medical professionals. The CareCHOICES Palliative Team works together with you to establish and accomplish the goals of maximizing comfort, managing symptoms and maintaining a holistic, personalized plan for care navigation.  

Regardless of whether a patient is in his or her own home, an assisted living facility or wherever else the patient may call home, the primary goal of palliative care is to improve the quality of life for the patient and family, to allow time to live every moment to its fullest.

Palliative services vary greatly, depending on the need and current condition of every patient, as well as the benefit coverage offered by each insurance company or medical group.

  • Guidance regarding complex treatment benefit or burden, emergency room admission, or re-hospitalization choices.

Eligibility

CareCHOICES Palliative Care is appropriate at any age and at any stage of a serious illness, and it is not dependent on a specific diagnosis or prognosis.  Our care may be provided from the time of initial diagnosis or throughout the course of the illness progression, in collaboration with your physician or team of medical specialists.

Your CareCHOICES Palliative Team will work with you while you are undergoing treatments that may cure or reverse the effects of the condition, or the team can guide you if you choose palliative care to assist with symptom management as the condition naturally progresses.

Insurance Coverage

Palliative care is typically considered a specialty service, and insurance coverage for the service varies greatly. However, a shift within the medical community has created a push for increased access to palliative care.  

CareCHOICES accepts all forms of reimbursement for services, and our admission specialists are available to explore your coverage options with you and your insurance company. We will make all of the calls so that you don't have to as we determine what supportive care coverage is available for you.

Interdisciplinary Team

Our CareCHOICES Palliative Team takes a holistic approach to palliative care.  A team of physicians, nurses, pharmacists, social workers, chaplains, certified home health aides, and volunteers work with you and your family to provide care, support, and guidance as you have need. Your Palliative Team will work closely with your current doctor for as long as you would like.  If you feel that you need more specialized care, our team will work with all medical professionals that you designate to be involved in your care process.

Your CareCHOICES Palliative Team will be available to you 24 hours a day, every day. Your comfort and knowledge of your care at any stage is our greatest priority.
